Welcome to The 2021 Mayor’s Cup Tournament

The 2021 Mayor’s Cup tournament will begin on Tuesday, August 31st and continue through Monday, September 6th.  Weekday games will be played in the evening and there will be no play on Sunday.  All Final games will be played on Monday, September 6th.

Out of State Teams or Teams coming from more than 2 hours away may check in two hours prior to game time at Tournament Headquarters (Angel Street Complex).  All Out of State Teams will need player cards.  

The team may be checked in by the coach, team manager or parent from the team. Once the team has checked in, the roster is frozen and no additional players may be added to the roster.

CHECK-IN PROCEDURES

ON-LINE – Local Teams:

Please complete the following steps to submit your roster for on-line check-in:

1. Prepare your official Affinity roster – instructions can be found at Utahavalanche.com
2. Save a copy of your Affinity roster as a PDF file
3. Email your completed roster and any guest player forms (if applicable) to the following email addresses:
   1. Boys’ team – 
   2. Girls’ team – 
4. Rosters must be received by Monday, August 30th.
5. If everything is in order, we will send you an approved roster via e-mail that you will need to make 6 copies of to bring to each game.

Tournament Only Players

Players not UYSA registered for the 2021-2022 season can play on a team in the tournament but must be registered as a tournament player through their region registrar. A $25 dollar fee will be required for this “tournament only” registration. “Tournament Only” players are not considered guest players and do not need the UYSA Specialty Player Action Form.

In Person - Out of State Teams:

• 6 copies of state sanctioned name roster
• Player Cards and coaches (with photos)
• Guest players must be listed on the roster.  A guest player form from the home state is required.
• Teams must check in at Headquarters (Angel Street Soccer Complex) before their first game.

PLEASE NOTE:

• Teams failing to meet Check-In requirements will not be eligible to play.

Teams must Check-In at least 2 hours prior to their first game (non-local teams)

Although not required at Check-In (according to a new UYSA policy) teams should have the following documentation at the field for every game:

• A medical release form OR UYSA Participant Registration Form with medical consent for EACH player, completed and signed (notary NOT required)
• If a player's age is questioned, a Birth Certificate must be provided before the next game (for out of state teams, a player card will constitute proof of age).  We can also reach out to UYSA if this is an issue.
